Transportation costs: Do you need transportation to get home after an operation, or for care away from home? The Health Insurance can take care of your transport costs if your state of health justifies it and under certain conditions.

It is your doctor who, if he considers that your situation warrants it, prescribes and supplements the form “Medical Prescription of Transport” and gives it to you. The medical prescription must be established before transport and, unless otherwise stated, must be given to the driver when getting into the vehicle. In the event of a medical emergency, the prescription can be filled after the event by a doctor from the healthcare facility in which you have been admitted.

Transport costs are reimbursed by Health Insurance, on medical prescription, for care or examinations appropriate to your state of health in the following cases:

• transport related to hospitalization (entry and / or discharge from the hospital), whatever the duration of the hospitalization (complete, partial, or outpatient);
• transport related to treatments or examinations for patients with long-term medical conditions who have one of the disabilities or deficiencies defined by the transport prescription reference system;
• transport related to treatments or examinations relating to an accident at work or an occupational disease;
• Ambulance transport, where the condition of the patient requires lying or being supervised;
• long distance transport (more than 150 km one way);
• serial transport (at least 4 transports of more than 50 km one-way, over a period of two months, under the same treatment);
• transport related to the care or treatment of children and adolescents in centers for early medico-social action (CAMSP) and medical-psycho-educational centers (CMPP).
• To go to a summons of the medical service of the Health Insurance, the regional commission of invalidity, of a medical expert (for a medical expertise of Social Security) or for an appointment at a supplier of apparatus approved for the supply of apparatus (ocular and facial prostheses, podo-orthoses, ortho-prostheses). In these situations, the convocation takes the place of a medical prescription and the mode of transport is indicated.
• To accompany a child under 16 or someone whose condition requires the assistance of a third party. In this situation, the doctor must specify the need for an accompanying person on the medical prescription.

The assumption of responsibility of certain transports requires to obtain the prior agreement of the medical service of the Health Insurance:

the taking into charge of certain transport, even prescribed by a doctor, requires the prior agreement of the medical service of the Health Insurance: this is the case, among others, long-distance transport (more than 150 km one way), mass transport, transport related to the care or treatment of children and adolescents in centers for early medico-social action (CAMSP) and medical-psycho-pedagogical centers (CMPP), and when the person transported needs to be accompanied or that it is a child under 16 years old. The transport costs of the accompanying person may also be covered by the Health Insurance. In this situation, the doctor must specify it on the medical prescription.

In practice :

Your doctor completes the “Request for Prior Approval / Medical Prescription of Transport” form and gives it to you.

Address strands 1 and 2 to the medical service of your health insurance fund, for the attention of “the medical adviser”.

Failure to reply within 15 days of sending your application is deemed an agreement: you may consider that your claim is accepted by your health insurance fund. Please note that this period does not apply in emergency case attested by the doctor.

In case of refusal, and only in this case, your health insurance fund will send you a notification letter.
